Job Summary Band 6 Community Mental Health Practitioner – a keyworker role within the West Lancashire Community Mental Health Team, part of Lancashire & South Cumbria NHS Foundation Trust. The position involves delivering assessment and therapeutic intervention to adults with severe, enduring and complex mental health needs within their homes or the community, supporting their recovery and improving quality of life. Key Responsibilities Act as a keyworker and manage a caseload of clients with varying mental health diagnoses. Assess patients, develop risk assessments, collaborative care plans and medication management. Provide care and support, including physical health monitoring and management of patient documents. Advocate for patients, offer support and signpost them to relevant services. Work as part of a multi‑disciplinary team delivering assessment and therapeutic intervention in the community. Qualifications & Experience Professionally Registered Mental Health Nurse or equivalent professional registered Allied Health Care Professional. Knowledge of mental illness and its impact on quality of life. Knowledge of risk formulation. Post‑qualification experience within mental health services, including acute services. Experience working with a full range of mental illness. Experience providing supervision and delivering teaching/training.
