Join our Children’s Community Health Team at HCRG Care Group, where you’ll provide high-quality Occupational Therapy assessment and intervention to children, empowering them and their families to achieve their full potential. As a Band 5 Occupational Therapist, you will play a vital role in a multidisciplinary team, supporting children with a range of developmental and performance challenges. You’ll work in partnership with families, carers, and other professionals to enable self‑management, independence, and well‑being in children’s daily lives. Base: You will be based at one of our Surrey‑wide office hubs, with the flexibility of hybrid working to support your professional and personal needs. This role is based in the community and will include working in schools & children’s home. Main duties of the job Carrying out high‑quality School Based Assessments, as part of a Multi‑Disciplinary Team (MDT) for children and young people Assess and provide tailored advice and interventions for children with diverse needs. Develop and implement individualised treatment plans in collaboration with families and carers. Recommend and support the use of specialist equipment for home and school settings. Liaise with professionals across health, education, social care, and voluntary sectors to ensure comprehensive care. Provide sensitive and effective communication with children and their families, adapting to their needs and circumstances. Contribute to team performance targets and participate in supervision, training, and team meetings. Assist with prioritising treatment goals for children with complex needs or terminal illnesses, ensuring appropriate discharge planning and referrals.
