About The RoleAs a Care Coordinator you will be an exceptional leader, the mark of your success is to make a positive difference to people's lives. You will spend each day assisting in the day-to-day operation of the Support Services, business and participate as required in multi-disciplinary assessment of service user needs.The successful Care Co‑Ordinator will be responsible for ensuring all care plans are reviewed on a 6 monthly basis, spot checks are completed on a 3 monthly basis, staff are reviewed quarterly, and staff annual appraisals are completed.This is an office-based role in Livingston, and you will be required to commute to services in and around the Livingston, Eilburn & Edinburgh area as well as working with service users with complex needs in our Bryson Court branch. You will be able to claim millage when out completing any reviews at 25p per mile, from office to service user and back to the office.Role ResponsibilitiesDirect line management for a team of Support Workers, including the management of their weekly workloadDefining special care needs based upon identification of specific clinical problems and to monitor and review as appropriateLiaising with external organisations in respect of developing a total package of care for a service usersReporting to a Branch Manager on a weekly basis on activity levels and outcomes for service usersProviding written reports of all complaints, accidents, incidents and untoward events in accordance with written policiesUndertaking “on call” duties as required by the branch to ensure a 24/7 service is provided.
