As part of the Lambeth South West Focussed Support Team, you will play a vital role within a Community Mental Health Team (CMHT) providing care and treatment to individuals with severe and enduring mental health conditions. Key Responsibilities Manage and coordinate care for a caseload of patients with complex mental health needs. Develop and implement personalised care plans to promote patient recovery and well-being. Conduct comprehensive risk assessments and apply effective risk‑management strategies. Work collaboratively with a multidisciplinary team to deliver holistic mental health support. Contribute to regular patient reviews, case discussions, and service improvement initiatives. Provide information, advice, and emotional support to patients and their families. Requirements Experience in care coordination and risk management within mental health services. Working knowledge of ePJS (Electronic Patient Journey System).
