Be aware of the needs of patients and their relatives and have knowledge of statutory and voluntary services. Be an effective member of a Nursing Team working within the community directorate. To be responsible for the caseload management and supervision of the nursing team, in the absence of the District Nurse/ Caseload Holder. The Trust was formed on 1st August 2010 through the integration of Croydon Community Health Services and Mayday Healthcare NHS Trust. Our main hospital site, Croydon University Hospital is one of the busiest in London.
