Be aware of the needs of patients and their relatives and have knowledge of statutory and voluntary services. Be an effective member of a Nursing Team working within the community directorate. To be responsible for the caseload management and supervision of the nursing team, in the absence of the District Nurse/ Caseload Holder. The Trust was formed on 1st August 2010 through the integration of Croydon Community Health Services and Mayday Healthcare NHS Trust.
