Neway International are seeking a Reablement Assessment Officer to join our client Cardiff Council. Location: County Hall, Cardiff (with community and homeworking as required) Hours: 37 per week (rota basis, 09:00 – 17:00, any 5 days out of 7) Start Date: asap End Date: 02 March 2026 (likely to be extended) Pay Rates: PAYE £18.85 per hour / Limited/Umbrella £24.56 per hour About the Client Cardiff Council’s People & Communities Directorate is committed to supporting adults to live independently in their own homes. The Community Reablement Team plays a vital role in assessing needs, developing strength-based support plans, and enabling individuals to achieve their reablement goals. The Role As Reablement Assessment Officer, you will assess the needs of individuals in their own homes and work with them to produce outcome-focused reablement plans. You will use a strength-based approach to maximise independence, make referrals to appropriate services, and ensure timely reviews. This role requires excellent communication skills, the ability to work flexibly across community, office, and homeworking environments, and a commitment to safeguarding and promoting independence. Key Responsibilities Assess service users’ health and social care needs, producing reablement plans in line with statutory requirements Work collaboratively with multi-disciplinary teams, including health and social care professionals Promote independence by recommending and ordering equipment, aids, adaptations, and assistive technology Review progress regularly, updating support plans and risk assessments as required Identify safeguarding concerns and take appropriate action in line with Council policy Communicate effectively with service users, families, carers, and partner agencies Maintain accurate records, assessments, and databases in line with procedures Provide on-call standby support as part of a rota when required Candidate Requirements NVQ/HSC Level 4 or equivalent qualification in Health and Social Care Management (or equivalent professional qualification) Eligible to register with Social Care Wales Experience working with clients with physical disabilities and/ or older people in a caring or rehabilitation environment Knowledge of equipment, adaptations, and manual handling techniques Strong assessment skills with a person‑centred, outcomes‑focused approach Excellent communication and organisational skills, able to manage workload independently Full UK driving licence and access to a car for work purposes Enhanced DBS required #J-18808-Ljbffr
