Overview Welwyn and Hatfield Integrated Care Team, winners of Hertfordshire Community Trust Leading Lights awards, is recruiting Band 5 Nurses to join an innovative, caring and agile Community Nursing team. The Trust has a Good CQC rating and is working towards Outstanding. Nursing care is delivered across the Welwyn and Hatfield locality to prevent hospital admission, facilitate discharge, deliver planned nursing intervention and manage long‑term conditions. Responsibilities Deliver high quality nursing care to patients in their own homes. Prevent admission to hospital and support timely discharge. Plan and implement nursing interventions for long‑term conditions. Work collaboratively with partnership organisations across East and North ICB, Hertfordshire Foundation Trust and Hertfordshire Community Trust. Support and develop skills within an award‑winning multidisciplinary team. Qualifications & Requirements Band 5 nursing qualification. Community nursing experience preferred; training available for suitable candidates. Ability to creatively problem‑solve and work on own initiative. Understanding of current professional and community issues. A full valid driving licence and access to a car for business use (unless a disability applies).
