Structural Clinical Nurse Specialist Band 6 Fixed Term (12 months) 2 MDT coordinators We have an amazing opportunity for two experienced nurses to join our world leading team at St Bartholomew’s Hospital. The post holders will deliver high-quality nursing care to patients with structural heart disease as well as patients with acute coronary syndrome. Responsibilities Have excellent communication with patients, their families, colleagues, and other members of the internal and external multidisciplinary team. Support the team coordinating MDTs for the structural team and the acute coronary care team and enacting the MDT plans. Work collaboratively in consultant-led clinics. Be a member of the ward-based Structural and ACS teams; reviewing patients pre and post intervention and providing support to nursing staff. Be an active member of the valve multidisciplinary/ACS team meeting; advocating for patients. Undertake regular teaching, training, and mentorship of the nursing team in the provision of care for structural heart disease patients/ACS. Actively participate in and promote research and service improvement and development. The MDT coordinator’s post is to provide dedicated support to the structural cardiology multi-disciplinary teams (“MDT”). The post holder will be expected to ensure the MDT runs smoothly and to time. Post holder will be required to live type at the meeting, which will be signed off by MDT Lead. They need to ensure all patients are discussed within agreed timescales, and treatment plans are actioned post MDT meeting. Post holder will use specialist knowledge of the patient pathway to proactively influence the tracking records using high levels of communication to expedite appointments/procedures as necessary and to resolve issues which could lead to delays in treatment.
