Job Summary Worcestershire Acute Hospitals NHS Trust (WAHNT) and Worcestershire Health & Care Trust (WHCT) are investing in teams to help provide robust care for our frail population. Working together, the two provider Trusts have roles for Geriatricians in acute and community settings. We are keen to hear from both senior clinicians and those just starting out. We can work with you to develop your area of special interest. Main Duties of the Job There are new academic opportunities appearing as WAHNT and WHCT have links with the University of Worcester with its Association for Dementia Studies as well as the medical school that opened in 2023 which is focusing on a curriculum emphasis on primary care, long term conditions and rurality. We have become a top priority for the trust and the Health and Care Trust to expand Acute Geriatrics / Frailty assessment on both sites (Worcestershire Royal and The Alexandra Hospital) to include Front Door Frailty Teams, Orthogeriatricians and Surgical Inreach Geriatricians, as well as having Geriatricians in the community who work with both WH & CH and the Acute Trust. Community working would be close with Worcestershire and Hereford STP which are responsible for 7 community hospitals which care for over 200 patients in total between them as well as 14 neighbourhood teams which link primary care, community care and social care. Outpatient Activity is available on 3 sites for speciality and general clinics including hot clinics. Job Description – Responsibilities Suitably experienced post-holders will be expected to provide a clinical service in the relevant specialty to patients of the Worcestershire Acute Hospitals NHS Trust and to General Practitioners. Post-holders will be expected to provide a service to patients of the Worcestershire Acute Hospitals NHS Trust, General Practitioners and to attend multi‑disciplinary meetings. The successful candidates will be expected to work within multi‑disciplinary teams in providing high quality clinical care. It is anticipated work will be undertaken on three sites and closer to the patient’s home. The Trust is seeking to extend services to GP surgeries/health centres etc and job plans may be revised in due course. Post holders will provide support to the A&E department and acute medicine to ensure timely appropriate care and patient flow through the hospital. Post-holders will provide consultation and advisory services to clinical colleagues in the Trust and Primary Care. Clinical opinion and support for teams in community hospitals: ward round / MDT. Clinical opinion and support for community neighbourhood teams. Clinical opinion and support for GPs supporting patients in their own homes and in care homes. Supporting clinical review of patients who are high users of community and secondary care. Face to face / phone advice / email / use of electronic patient records. Mentoring generalist clinical staff and supporting enhanced training. Encouraging a Comprehensive Geriatric Assessment approach to management of older people living with frailty in both stable and unstable situations. Encouraging an anticipatory planning approach to managing acute deteriorations including contingency planning and inappropriate admission avoidance. Supporting management and transfer decisions in all care settings to ensure right place of care. Supporting teams in managing the perceived risks of care outside an acute setting. Supporting the education of all those managing Older People living with Frailty and frailty related conditions e.g. GPs ANPs AHP nurses and care staff. Post-holders will attend cross‑county & cross‑site meetings. To collaborate with colleagues to deliver a high quality, timely service and to work towards meeting the Trust’s and departmental objectives. Post holders will if relevant, participate in the on‑call rota to ensure that both emergency and urgent services are provided out of normal hours, to be shared equally with consultant colleagues in post. The Trust would expect consultants whilst undertaking on‑call duties to attend the hospital site to review patients when clinically necessary. The consultant on call must be prepared to attend willingly whenever requested to do so by a member of their medical and nursing team. The post‑holder will develop close collaborative links with Clinicians working in his/her area of specialist expertise and work to develop services to patients in these areas to the highest standard, according to departmental protocols and Trust guidance. To include development of diagnostic and where appropriate therapeutic protocols and guidelines, where appropriate this will involve active participation with Primary Care colleagues. The post‑holder will participate in the Trust’s annual appraisal as described by DOH and leading to revalidation and job planning process. The post‑holder will seek approval for any absence from the Trust such as annual leave, professional leave, study leave & other leave and make available this record at your job plan review. Consultants will provide cross cover for periods of annual and study leave. Person Specification Qualifications – Essential Full GMC Registration with a Licence to practise. Success in Intercollegiate Specialty Examination or overseas equivalent. Hold MRCP or equivalent qualification. Qualifications – Desirable Higher degree. Entry on Specialist Register in General Medicine / Elderly Medicine (or CCT/CESR expected within 6 months of interview date). Experience – Essential Clinical training and experience equivalent to that required for gaining (UK) CCT/CESR in General Medicine and Geriatric Medicine. Ability to offer expert clinical opinion on range of problems both emergency and elective within specialty. Ability to take full and independent responsibility for clinical care of patients. Expertise in subspecialty of Orthogeriatrics where specified. Other Requirements – Essential An understanding of the current NHS environment, particularly in relation to reforms, initiatives and issues. Post holder will attend cross‑county and cross‑site meetings. Able to meet the travel requirements of the job and return to the hospital within 30 minutes when on call. Management and Administrative Experience – Essential Ability to organise and prioritise workload effectively. Ability to advise on efficient and smooth running of specialist service. Ability to organise and manage outpatient priorities. Ability to manage and lead the general medical, elderly care and multi‑disciplinary teams. Experience of audit management and conducting clinical audit. Management and Administrative Experience – Desirable Ability to manage and lead specialist unit and working parties as appropriate. Ability to use evidence based and clinical audit to support decision making. Teaching Experience – Essential Ability to teach clinical skills to medical, nursing staff and other disciplines. Experience of teaching clinical skills to undergraduates and postgraduates. Teaching Experience – Desirable Ability to supervise postgraduate research. Research Experience – Essential Ability to apply research outcomes to clinical problems. An awareness of current specialty specific developments and initiatives. Research Experience – Desirable Publications in relevant peer‑reviewed journals in the last 5 years. Evidence of having undertaken original research. Personal Attributes – Essential Ability to work in a team. Enquiring, critical approach to work. Ability to communicate effectively with patients, relatives, GPs, nurses and other agencies.
