Job Overview We are seeking to recruit enthusiastic, highly motivated Band 5 Occupational Therapists to work within our Integrated Neighbourhood (INT) in South Gloucestershire. You will have the opportunity to deliver exceptional community-based therapy, improve the quality of life for service users and commit to rehabilitation in a home environment. You will have the chance to develop specialist community skills, enhance multidisciplinary working, support and supervise other team members and develop key leadership skills. The Sirona Integrated Neighbourhood Teams (INT) are an innovative and dynamic service. We provide therapy and support to patients in their place of residence to maximise functional potential and support independence within their homes. Responsibilities Be responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell, often with complex needs arising from the frail elderly cohort, with the support of senior staff. Use clinical reasoning skills to assess, plan, implement and evaluate patient‑centred intervention programmes using an evidence‑based practice approach, promoting high quality and harm‑free care. Collaborate and negotiate with service users, carers and family in identifying achievable goals as part of the overall care plan, maximizing quality of life and independence. Facilitate appropriate referrals to other services. Work within a multi‑disciplinary team to provide integrated skills and share knowledge, providing specialist skills supported by a robust competency framework. Use occupational therapy skills, techniques in functional and standardised assessment & interventions as an autonomous practitioner. Assess and arrange the provision of equipment, ensuring correct fitting and good working order; report any issues appropriately. Supervise members of the team including peers, students and unregistered workforce in occupational therapy competencies. Maintain accurate, informative and legal records. Actively participate in training; you may assist in the delivery of training. Work Hours Flexible service covering extended hours operating 7 days a week, all year round. Shifts are a mixture of 8‑6 & 10‑8 (four days a week). Weekends 1:3/4 equivalent.
