Central and North West London NHS Foundation Trust is recruiting a highly skilled, motivated and passionate Band 6 Memory Service Nurse for the Memory Assessment Service within the Harrow Older People Community Services. The post holder will work as part of the Harrow Older People and Healthy Aging Service, delivering community mental health assessment and intervention, providing high quality initial assessment, investigations and ongoing review for older adults with memory problems. Key Responsibilities To be responsible for the flexible management of a defined caseload designed to meet the individual needs of the client and the carer’s. To maintain high standard of individualized client care, and give guidance on how clients can improve their mental, emotional and physical well‑being of clients/carers. To provide a specialist and holistic assessment of client’s memory and initiate any further investigations in the absence of the Clinical Lead and partake in the multidisciplinary team meeting. To develop and maintain a high standard of practice. Supervising, monitoring and administering medication as necessary, ensuring the policy for carriage and administration of drugs is strictly adhered to. (RMN/RGN’s only) Monitoring compliance and observing effects and side effects of medication, regularly updating knowledge on medication. Ensure that all views of Service Users are effectively sought, channelled and acted upon, including the efficient actioning of complaints in accordance with the Trust policy. To act in a manner to raise standards of care and make the services culturally sensitive, relevant and accessible to meet the needs of a diverse population. To be conversant with the Safeguarding Adults procedures and alert to signs of possible abuse. Person Specification Education & Qualification RMN Evidence of continued professional development in post‑registration qualifications Desirable Criteria A post registration qualification in Older People Experience Essential criteria Experience of working as member of a multi‑disciplinary team Experience of working with older adults experiencing memory problems and dementia Desirable criteria Experience of managing/supervision of staff. Experience of working in a community setting Knowledge Essential criteria Knowledge of CPA, Risk Assessment and Risk Management Processes Knowledge and understanding of the needs and rights of service users and their carers. Knowledge of ethical/legal requirements within mental health care. Knowledge and understanding of legislation relating to the care of older adults with mental health problems in the community #J-18808-Ljbffr
