Community Nurse - Community Response Team (CRT) Stratford and Warwick An exciting opportunity has arisen for a dynamic, forward‑thinking, and self‑motivated individual to join our Community Response Team based over two sites in Leamington and Stratford. The Community Response Team operates across the hours of 08:30‑21:30, seven days a week, including weekends and bank holidays. Shifts are allocated pro‑rata, offering a mix of early, late and long days. To be successful with this post, you must live in the UK and have a full NMC registration. You must also possess a valid driving licence and have access to your own car for this role. Mileage will be paid for work‑related travel. As a Community Nurse working in CRT you will assess, identify and manage patients’ needs who have experienced a deterioration in their health. You will have a varied caseload seeing patients with long‑term conditions, falls, acute illness, supporting patients at the end of life, and providing holistic, comprehensive care in their own homes. The Community Response Team’s aim is to prevent hospital admissions and facilitate timely hospital discharges. You will also work closely with the multidisciplinary team—including therapists, GPs, social services, urgent response and many others—collaborating with other community service teams to deliver seamless care across the locality within an unpredictable and unscheduled workload. The community staff nurse is a member of the Integrated Health Team, undertaking professional duties in the client’s homes and clinics to ensure the provision of high‑standard nursing care and health promotion. Working across professional boundaries and providing comprehensive health care sensitive to the needs of the local population is essential. Join a Trust rated Outstanding by the CQC.
