Merton Council is looking for Reablement Officer for Reablement Team ( 1 Year fixed term).This role involves carrying out visits to customers who are discharged from hospital, completing risk assessments and care planning for customers in the Borough of Merton. You will work closely with all staff within the organisation to ensure care is delivered according to care Act requirements. You will work closely with the integrated Health sector and professionals.The shift pattern is 7 days a week rolling rota, including bank holiday and weekends. Working pattern vary from a morning shift from 7 : 30 am to 3 : 30 pm and late shift from 1 : 00 pm to 9 : 00 pm.Also within this role you will manage support workers day to day work and development.This job includes Essential car user allowance and weekend enhancement.Experience in care sector is an advantage but not essential.You need to have a UK driving licence and the use of a car to apply for this job. #J-18808-Ljbffr
