Eden Brown Synergy is currently recruiting for an experienced Occupational Therapist (Band 6 / equivalent) to join the Integrated Reablement Service within Adult Social Care at Tower Hamlets. The Role: You will work with service users in the community for up to six weeks on a rehabilitation programme, supported by in-house Reablement Officers (ROs) who deliver reabling care under the direction of the OT. The primary goal is to prevent, reduce or delay the development of long-term care needs. This role offers a varied caseload, including both hospital discharges and community referrals, providing a rewarding and diverse work environment. Key Responsibilities: Undertake assessments and develop reablement plans tailored to individual needs Provide clinical direction and oversight to Reablement Officers Support hospital discharges and community rehabilitation cases Complete risk-assessed home visits as required Work collaboratively within a strong, supportive multidisciplinary team Requirements: Minimum 2 years post-qualified experience as an Occupational Therapist Experience in physical rehabilitation and case management in a busy setting Must be London-based to manage urgent case-related events Not suitable for newly qualified or Band 5 OTs Working Pattern: Monday to Friday with flexible hours Some remote working available Expected to be office-based at least two days per week Benefits of workin...
