Job Overview Exciting opportunities have arisen for new Community Rehabilitation Assistants to join Poole Intermediate Care Service. Community Rehabilitation Assistants will be part of a Multi-disciplinary Team caring for patients with an illness in their own homes, to prevent hospital admissions and facilitate complex discharges from hospital. They will work alongside qualified Occupational Therapists, Physiotherapists, Nurses, Therapy Technicians and wider locality teams. Good communication skills and a caring attitude are needed. Key Responsibilities To contribute to the aims of the service by working to prevent avoidable hospital admissions and to support early supported discharges from acute and community hospital units. To contribute to the objectives of the service by working to increase independence, reduce dependence on long-term health or social care and, where achievable, to avoid admissions to long-term care. To work as part of an integrated, multi-disciplinary team delivering outcome-based packages of intermediate care to users of the service. To contribute to the planning, organisation and delivery of packages of integrated, intermediate care, as agreed by a qualified practitioner. Key Requirements Shift working is required: the service operates 365 days a year 08.00 to 20.00. Successful candidates will need to work some weekends and 34 hours per week or less are available. Use of a car to visit patients in their own homes is required – car owner/driver essential subject to the provisions of the Equality Act 2010. Basic IT skills and NVQ level 2/3 would be advantageous. Employment requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.
