We are seeking a skilled Pharmacy Technician to join our team at Trent Meadows Medical Practice. In this role, you will be essential in supporting our pharmacy operations and ensuring that patients receive the medications they need safely and effectively. Main duties of the job Key Responsibilities: Support medicines reconciliation and verification to ensure accurate patient medication records. Assist with clinical medication reviews and Structured Medication Reviews (SMRs). Maintain and update patient medication records and databases. Provide medicines-related information and support to patients and healthcare staff. Assist in audits, quality improvement, and service development initiatives. Collaborate with multidisciplinary teams to support medicines management. Ensure compliance with legal, professional, and organisational policies related to medicines. Provide administrative support to pharmacy services, including referral management and appointment coordination. Provide excellent patient care by answering queries and offering medication advice. Job responsibilities To work as part of a multidisciplinary team in a GP practice or Primary Care Network (PCN) to support the delivery of safe, effective, and efficient medicines optimisation. The Pharmacy Technician will assist in repeat prescription management, medicines reconciliation, audit, and patient education, working under the supervision of a Clinical Pharmacist and in collaboration with GPs, nurses, and other healthcare professionals. Support the practice in implementing safe, evidence-based, and cost-effective prescribing. Undertake medicines reconciliation following hospital discharge, outpatient appointments, or other care transfers. Review repeat prescriptions, ensuring accuracy, adherence to formulary, and compliance with prescribing policies. Identify and flag patients for medication reviews or optimisation by a clinical pharmacist or GP. Support audit and quality improvement initiatives related to prescribing and medicines safety. Assist with the management of high-risk medicines and monitoring requirements. Repeat Prescriptions & Workflow Management Process and authorise repeat prescriptions in accordance with practice protocols. Ensure appropriate clinical checks, quantities, and review dates are applied. Communicate with community pharmacies, care homes, and patients to resolve prescription queries. Streamline prescription processes to reduce GP administrative workload. Provide advice to patients on safe and effective use of medicines within scope of practice. Promote adherence, waste reduction, and understanding of medication regimens. Support patients recently discharged from hospital or with long-term conditions. Participate in patient education sessions (e.g., inhaler technique, medicine reviews). Work closely with the Clinical Pharmacist, GPs, nurses, and admin staff to improve prescribing systems. Liaise with community pharmacies, secondary care, and other health providers to ensure continuity of care. Participate in multidisciplinary meetings, practice development sessions, and audits. 5. Governance, Compliance & Quality Maintain accurate, up-to-date patient records on EMIS, SystmOne, or equivalent. Ensure compliance with GPhC standards, data protection (GDPR), and confidentiality policies. Contribute to the practices medicines management policies and protocols. Support the delivery of QOF, IIF, CQC, and local prescribing targets. 6. Training & Development Participate in continuing professional development (CPD) and mandatory training. Engage in peer support and supervision sessions with clinical pharmacists. Support the education and training of other staff within the practice where appropriate. Work towards completion of the Primary Care Pharmacy Education Pathway (CPPE) or similar programme. The postholder will work independently within their scope of competence and under appropriate supervision from a registered clinical pharmacist. The role may evolve as the PCN develops and as national priorities change. Person Specification Qualifications NVQ Level 3 in Pharmacy Services or equivalent. Experience in community, hospital, or primary care pharmacy. Strong communication, IT, and organisational skills. Understanding of medicines governance and primary care prescribing. Understanding of primary care and NHS structures, including the role of PCNs. Experience in general practice or PCN. Knowledge of EMIS system Completion or enrolment on CPPE. Excellent communication and interpersonal skills able to liaise effectively with patients, GPs, pharmacists, and other healthcare professionals. Competent in IT systems, including Microsoft Office and GP clinical systems. Strong attention to detail and commitment to patient safety.
