Intermediate Care Physiotherapist The closing date is 04 December 2025 *** PLEASE NOTE :- This post is currently being advertised for internal staff working at Bridgewater and WHH *** This is an exciting opportunity for a Band 5 to progress or a current Band 6 Physiotherapist to develop skills in an integrated community intermediate care service. The role encompasses assessment and rehabilitation functions that are carried out predominantly in the patient's home or in one of the designated bed bases. Working under the Discharge to Assess model, the physiotherapist will utilise core professional skills alongside additional generic skills to provide a holistic assessment of the patient's health and care needs to ensure a safe transition to home. Following assessment, the occupational therapist will provide rehabilitation to patients in their home or in a bed base. The post is rotational, you will be given responsibility for providing physiotherapy for a designated area, including community rehabilitation, bed-based rehabilitation or D2A services. Interviews will be held 15th and 16th December 2025. Main duties of the job The successful candidate will have clinical responsibility for a designated caseload of patients who are under the care of the intermediate care service. They will provide Physiotherapy rehabilitation focused assessment and interventions to these patients as well as directing therapy assistance and social care support workers to support patients in completing rehabilitation tasks between therapist visits. The successful candidate will be responsible for supervision of junior staff and students within the service. They will also support the clinical lead with audit and service development tasks as appropriate. About us Flexible working will be considered for all roles. Job responsibilities Job Purpose: To undertake specialist assessment of patients within their own or bed base units with acute/unstable conditions who are referred to the Intermediate Care Service as an alternative to hospital admission. To undertake Physiotherapy assessment, diagnosis and deliver treatment programmes for adults with intermediate care needs in the Warrington area. To have continuing responsibility for a defined caseload of patients who have been identified as being at risk of inappropriate hospitalisation. To actively participate and contribute to multidisciplinary team working also liaising with other professional agencies to promote and plan rehabilitation and care. Duties and Responsibilities Thislist of duties is not intended to be exhaustive, but indicates the main areasof work and may be subject to change after consultation with the post-holder tomeet the changing needs of the organisation (a) Clinical To undertake comprehensive and holistic physiotherapy assessments of patients including those with diverse presentations, multiple pathologies and complex needs, in a variety of settings. Based on assessment, to develop and deliver individualised and graded physiotherapy treatment programmes, that are regularly reviewed and evaluated to ensure intermediate care outcomes are met. To utilise advanced analytical skills and clinical reasoning and apply to the physiotherapy process in a person-centred approach. To review and re-evaluate the patients response to physiotherapy interventions to ensure that goals are met within the time limits of intermediate care. Based on the outcome of the assessment and treatment programme, to formulate appropriate and timely discharge strategies. To modify treatment skills as appropriate to the service user's environment. To assess and treat patients, in their own environment, without prejudice and in conditions hazardous to health with the potential to cause harm. To provide planned and spontaneous advice to patients, informal and formal carers, relatives, junior staff and fellow professionals promoting understanding of the aims of Physiotherapist and ensuring a consistent approach to patient care. To work autonomously within scope of practice and to consult more experienced team members as necessary. To communicate effectively with patients, carers, relatives, colleagues and junior staff, assistants etc in order to progress specific rehabilitation and treatment programmes. To deal with distressing and/or emotional situations with tact and sensitivity. To implement best practice based on evidence and clinical knowledge. To liaise with all members of the multi-disciplinary team both formally in the MDT and informally as appropriate, to facilitate progress through intermediate care. To facilitate management of manual handling risks in accordance with the Bridgewater policy. To be responsible for maintaining and managing accurate and comprehensive intervention records inline with professional and local standards. (b) Clinical Management To accept clinical responsibility for an identified caseload and to prioritise this effectively, according to clinical and service needs and time available. To manage the clinical caseload with autonomy, guided by Bridgewater policies and procedures, professional codes of practice and departmental working practice. To communicate with a wide range of professionals both within the Trust, and in external agencies including primary care, the acute trust, social care, voluntary organisations etc. To engage patients in the consent process to facilitate informed compliance with physiotherapy interventions. To attend relevant clinical meetings, safeguarding meetings and case conferences as appropriate. To provide training for own discipline, and to be involved in the training of all team members/disciplines and students. To supervise less experienced staff, students and assistants and delegate specific responsibilities where applicable. To provide accurate statistical information in line with local standards and the professional body. To participate in clinical audit relevant to the post. To adhere to the Trust lone worker policy at all times. To comply with organisational and departmental policies and procedures and be involved in reviewing and updating as appropriate. (c) Professional Development To work to the standards of professional practice as described by professional body and the HCPC. To be responsible for maintaining own competency to practice through Continuing Professional Development activities as identified by the HCPC. To continually evaluate clinical practice and modify approach in line with team and organisational objectives. To prepare for and actively participate in the Performance Development process both of yourself and others. To update clinical knowledge and skills and evidence based practice as identified through the PDR process utilising appropriate learning opportunities.
