As a Community Care Officer, you will assess the needs of adults requiring social care, coordinate essential support, and empower them to live independently. In This Dynamic Role, You Will Assess Needs: Conduct thorough needs and risk assessments in line with the Care Act 2014 and Mental Capacity Act 2005. Build Relationships: Foster strong connections with individuals and their families to understand their unique situations. Coordinate Support: Work collaboratively with social workers, occupational therapists, and other agencies to provide comprehensive care. Connect Communities: Link individuals with local services and resources to promote self‑directed support. Respond to Inquiries: Address questions and concerns from clients and their families with empathy and professionalism. To Excel In This Role, You Should Have A relevant qualification in social care or a related field. Experience in adult social care or community support. Knowledge of legislation affecting social care, including the Care Act 2014 and Mental Capacity Act 2005. Strong assessment and analytical skills.
