We have GP Vacancies at our Health Board Practice, West End Medical Centre, in Conwy County; Westend Medical Centre is situated in Colwyn Bay, with a list size of 18,400 patients. Newly merged from Practices within the same building, West End Medical Centre is focused on delivering a social model of care and has a multi‑Disciplinary team including GPs, Nurses, Physiotherapists and Pharmacists. The practice is also moving towards recruitment of other health care professionals such as Advanced Practice Paramedics and Physicians Associates. Main duties of the job In general, the post-holder will be expected to undertake the normal duties and responsibilities as associated with GP work within primary care. In accordance with a managed practice timetable, the post-holder will make him / her – self available to undertake a variety of duties including guardian y consultations, telephone, video, and email consultations, and queries, visiting patients at home, providing advice to others, and dealing with queries, paperwork, referrals and communication in a timely manner. Make professional, autonomous decisions in relation to presenting problems, inc. undifferentiated and un-diagnosed problem, where the self‑referred or referred from other healthcare workers within the organization, with due referral to best practice and evidence‑based guidelines. Screen patients for disease risk factors, early signs of illness and promote public health interventions. Admit patients requiring triage and referring to the appropriate care providers as appropriate. Recall and manage a condition in a temporary suggestion notation and decision notest to agree standards. Collect data for audit purposes.
