Job Summary Medicines Management Assistant (Band 3) – 37.5 hours per week – Permanent – Support Technicians and Pharmacists in Acute Medical Admissions at Tameside and Glossop Integrated Care NHS Foundation Trust (T&G ICNHSFT), providing Medicines Management services and, on occasion, supporting the Emergency Department and Same Day Emergency Care (SDEC). The post holder works under the supervision of a Pharmacy Technician to assist in the delivery of pharmaceutical care to patients within the Trust through a safe, efficient and cost effective medicines management programme. The role involves direct patient contact. Responsibilities Optimise patients’ own medicines brought into hospital through direct or remote discussion with patients, family relatives and carers. Communicate the medicines optimisation strategy to the multidisciplinary team. Ensure patients, relatives and carers understand the advantages of the Trust’s Medicines Management Programme, using persuasive skills and knowledge of how to overcome barriers. Obtain consent for the patient’s Summary Care Record to be viewed, ensuring compliance with access regulations and patient confidentiality procedures. Record medication information onto the prescription sheet or, in the future, the electronic system. After appropriate training and competency assessment, participate in the medicines reconciliation service at ward level – ordering medication, assessing patients’ own medicines and the need for compliance aids. Provide pharmacy‑related information to the multidisciplinary team. Qualifications & Experience Previous experience of working in a busy hospital or community pharmacy setting. Effective verbal and written communication skills with patients and service users. Ability to prioritise own workload within organised rotas. Accuracy and carefulness in work. Capacity to work under pressure in a busy department and as part of a large team. Respect for colleagues, other disciplines and the public. Desirable Qualifications Familiarity with keyboard skills required for departmental computer systems.
