Go back Cambridgeshire and Peterborough NHS Foundation Trust Healthcare Assistant The closing date is 05 November 2025 We are looking for an Integrated Care Worker to join our Ely and surrounding villages Community Nursing Team We have an exciting opportunity to onboard a new member to our Nursing team, working with us, you will be fully supported to gain new skills and experience needed to deliver high quality care to patients in their own home or residential setting in and around the area. We provide a service seven days a week, working closely with other community services and Primary Care Networks to provide high standard holistic care to the population of Cambridgeshire. You will have the opportunity to develop yourself, with extensive learning and development opportunities. Main duties of the job Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. As a Health Care Assistant you will work closely with the nursing team to follow a plan of care in deliver high quality, best practice care to adults with a range of complex health needs within their own homes. Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy. Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals. Work under guidance on specified tasks, taking direction from registered and non-registered staff as appropriate, with regular clinical support and supervision by a qualified practitioner. Promote patient independence and self-management Provide and maintain a safe, comfortable and dignified environment for all patients/clients and carers To practice the principles and philosophy of individualised patient care in accordance with Trust Policy, procedures and regulatory body's guidelines. To maintain appropriate and timely documentation of each patient contact (ensuring these are recorded on SystmOne within 24hours). Plan and prioritise own visits to delegated patients, (and ensure these are documented on SystmOne) . Person Specification Education / Qualifications BTEC/NVQ Level 3 in health and Social Care. Experience Experience of working in a healthcare environment or care environment with older people Has worked as a Healthcare Assistant in the past. Skills & Abilities Able to demonstrate commitment to high quality care and service provision Ability to record messages and information in an accurate and concise way. Work flexibly to accommodate patient/service needs Experience of using SystmOne Physical Requirements Able to travel around the county at short notice using own vehicle and holds a valid DVLA Driving Licence Other Able to work shifts and unsocial hours when required Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
