Role Overview Are you passionate about making a positive and lasting difference for young people transitioning into adulthood in Hull? We are looking to appoint a Grade 9 Specialist Transition social worker to be part of a newly formed Preparation for Adulthood Team (PFA) based within the High Needs Team in Adult Social Care. This role supports younger adults with a diagnosis of Learning Disability, Autism or Mental Health, working as part of an integrated multidisciplinary team to provide high‑quality social work practice. Key Responsibilities You will: Ensure individuals have a solution‑focused, strength‑based plan towards achieving their goals and maintaining independence throughout their journey transitioning into adulthood. Work creatively and collaboratively with younger persons (sometimes from the age of 16) and partners in Children’s services to provide a consistent approach within a social work interface for their transition into adult services. Provide multidisciplinary input alongside Children’s Disability Teams, Education and SEND, CAMHS, health professionals, probation, criminal justice, housing and commissioning managers to deliver the best outcomes for younger persons and create a positive introduction to our Adult Social Care culture. Support the delivery of high‑quality social work activities, assessment and planning, initiating, monitoring, developing and providing outcome‑driven services in adult social care. Make complex decisions for vulnerable individuals and work autonomously within the team context. Qualifications and Experience Qualified social worker registered with Social Work England. Experience in assessment and planning, initiating, monitoring, developing and providing outcome‑driven services in adult social care. Demonstrated passion and value for working with young adults, strong commitment to quality outcomes and the ability to discuss what good social work practice looks like. Capacity for honest reflection and openness to learning. Ability to establish professional, effective working relationships with a range of partners, colleagues, adults, families and carers. Experience supervising staff and supporting ASYE social workers, including mentoring apprentices and trainee social workers. Experience with Court of Protection work, community deprivation of liberty and 21A challenges. Knowledge of the criminal justice system. Experience of promoting autonomy and implementing personalised safeguarding in a multidisciplinary arena. Understanding of the complexities involved with vulnerable younger persons.
