We are looking for a Senior Registered Adult Nurse to work within a community integrated team as part of a multidisciplinary team, full or part time, with a minimum of 28.5 hours per week. In this post you will be covering Tendering East neighbourhood. The service provides 24/7 nursing and therapeutic input to patients within their own homes with the aim of returning them to maximum independence, by actively promoting self‑management on all activities of daily living. You will provide a case‑management approach to care for an identified number of patients working closely with GPs, the ICTs and other statutory and voluntary agencies. This will be achieved by providing advice, information and education to patients, family and carers with the aim of enabling patients to enjoy maximum independence, health and wellbeing. You will support all members of the team by being an effective team member whilst working on your own initiative. You will have a commitment to collaborative working and support operational managers to resource the service and understand the professional and clinical decisions which underpin procurement decisions. You will be responsible for the expert assessment, planning, implementation and evaluation of programmes of evidence‑based nursing care to meet complex health‑care needs to individuals and their carers within the defined caseload across the community setting. You will be expected to provide ongoing nursing care and support to patients and carers, promoting the role of the nurse and clinical excellence. You will support any adult who requires care at home through a diverse range of pathways which support wellness and promote independence where the patient’s care can only or is best delivered within the home environment. Responsibilities Cover Tendering East neighbourhood. Deliver 24/7 nursing and therapeutic input to patients in homes, promoting self‑management of daily living. Provide case‑management for a defined caseload alongside GPs, ICTs, statutory and voluntary agencies. Advise, inform and educate patients, families and carers to support independence, health and wellbeing. Act as an effective team member while working independently, supporting collaborative working and operational resource planning. Carry out expert assessment, planning, implementation and evaluation of evidence‑based nursing programmes for complex healthcare needs. Provide ongoing nursing care and support, promoting clinical excellence. Support adults requiring care at home via diverse pathways that promote independence and wellness.
