Static Physiotherapist – Rapid Response Team Working Full Time: 37.5 hours per week. Job Overview This is an exciting opportunity to work within the Rapid Response Team at Countess of Chester Hospital. It is an expanding service that provides support to patients both in the acute and community setting. Key Responsibilities Permanent post within the Rapid Response Team, covering a 7-day 8am‑9pm service. Based at the Countess of Chester Hospital or one of the three Community Response Hubs (Ellesmere Port, Chester, Tarporley) with an expectation to work across these areas as needed. Provide car driving, hold full licence, business insurance, and have access to a car during working hours. Support the Rapid Response Team therapy caseload across all areas, covering the entire patient discharge pathway. Assess and discharge patients in SDEC and A&E. Complete discharge visits in the community within 2 hours of hospital discharge to ascertain patient needs. Manage a large and varied therapy caseload within the community. Utilise experience in core physiotherapy rotations and be confident in discharge planning and autonomous working within the community setting. About the Trust The Trust provides services to West Cheshire and Welsh patients, working collaboratively within the wider Cheshire and Merseyside Health and Care Partnership. Services are delivered from three locations: Countess of Chester Hospital, Ellesmere Port Hospital, and Tarporley War Memorial Hospital.
