Job Summary Working Full Time: 37.5 hours per week This is an exciting job opportunity to work within the Rapid Response Team. This is an expanding service which provides support to patients both in the acute and community setting. This post is a permanent post within the Rapid Response Team which would be suitable for an experienced Physiotherapist with a wide knowledge base and confident and competent in timely discharge facilitation. This post is to cover a 7-day 8am-9pm service. The successful candidate will be based either at the Countess of Chester Hospital or one of the 3 Community Response Hubs (Ellesmere Port, Chester, Tarporley) with an expectation to work across these areas as the service necessitates. The successful candidate will need to be a car driver with a full licence, business insurance and access to a car during their working hours. Main Duties The successful candidate will be based at one of the locality hubs but will work across all areas to support the Rapid Response Team therapy caseload. The entire patient discharge pathway will be covered within this role. From assessing and discharging patients in SDEC (Same Day Emergency Centre) and A&E, completing discharge visits in the Community within 2 hours of hospital discharge to ascertain patient's needs, through to managing a large and varied therapy caseload within the community. The successful candidate will need to have had experience in the core Physiotherapy rotations in addition to being confident and competent in discharge planning and autonomous working within the Community setting.
