Job overviewWe are looking for an innovative, skilled, compassionate Learning Disability Nurse for our new Specialist Community Learning Disability Service as a Band 5 Community Learning Disability Nurse.
This is an exciting opportunity to join our Nursing Team as we embark on a journey of change and work to implement a new model of working. This will provide an opportunity to shape the Nursing Team and the service we offer our community as we move forward.
The service offers standard and enhanced pathways and will be more responsive and accessible, providing specialist health assessments, support, and care for adults aged 18 and over. Care will be provided in a community setting, close to people's homes.
The Nursing Team have implemented the Moulster and Griffiths Nursing model, a specialist evidence-based, person-centred model to provide holistic care to those accessing Nursing support. Positive Behavioural Support also underpins person-centred care, in line with National agendas.
As a service, with a wide range of professions, we work, learn and combine our expertise together, providing high-quality integrated community, mental health, and learning disability services across Sheffield and are looking for someone who shares our values and is committed to helping improve the lives of the people we support.
Our transformation has been co-designed and co-produced with stakeholders both internal and external to our organisation, alongside experts by experience, carers and service users.
Main duties of the jobThe post holder will:
- Hold a clinical caseload as allocated by the Team Manager
- Deliver a nursing service within the framework of a multi-disciplinary team and provide specialist learning disability services to adults with learning disabilities and complex healthcare needs and/or behaviours that may challenge others
- Contribute to the training and development of other health services, agencies and individuals involved with people with learning disabilities.
- Keep up to date with current trends, or developments, that affect services offered to people with learning disabilities.
- Undertake post-registration training and continue professional and personal development.
- Report any complaints, compliments and service deficiencies in accordance with Sheffield Health and Social Care NHS Foundation Trust policies and procedures.
- Be professionally and legally responsible and accountable for all aspects of work.
- Develop a working knowledge of relevant procedures including Mental Capacity Act and other legal frameworks in line with government and local policy.
- Be flexible to work between the hours of 8am – 8pm Monday to Friday.
At SHSC, we prioritise the well-being and safety of both our service users and employees. It is our policy and a condition of employment that all employees must join the DBS Update Service. This service carries an annual fee, which employees are responsible for paying up front. However, employees may claim this cost back through the Trust's Employee Expenses Reimbursement Policy. For those requiring a Basic DBS check, it is necessary to register for a DBS Online Account.
Detailed job description and main responsibilitiesProfessional
- To ensure the process of referral, assessment, planning, intervention and evaluation of care is efficiently and effectively delivered with clear quality outcomes.
- To provide comprehensive specialist assessments to individuals with a learning disability working across both the standard and enhanced pathways to meet an individual's needs.
- To develop a plan of care as appropriate to meet the health needs of the person with learning disabilities.
- Work autonomously and carry a caseload of service users who need specialist intervention which is agreed and reviewed through regular supervision, maintaining regular visits and up to date assessment and care planning for all active clients.
- To ensure the effective application of the Mental Capacity Act 2005.
- To carry out effective risk assessment and implement risk management plans as per Sheffield Health and Social Care NHS Foundation Trust's policies.
- To work with other members of the team on developing the knowledge and skill base around Positive Behaviour Support.
- To contribute to assessment and management plans for service users with high risk clinical needs to ensure safety of service users, carers, staff, and the general public.
- To be responsible for the correct administration of prescribed medication in line with care plans.
- To undertake physical observations as part of baseline monitoring including blood pressure, temperature, respirations.
- To adhere to the Nursing and Midwifery Council Code of Conduct and to Sheffield Health and Social Care NHS Foundation Trust policies.
- To attend meetings that are appropriate to care delivery and personal development.
- Demonstrate ability to work as part of a team but is also competent and confident to work independently, providing expert advice to all allied health professionals.
- Raise the profile of people with learning disabilities and their health needs within Primary Care and enable greater uptake of health screening services.
- Work in partnership with GP practices and other relevant professionals identified across the health community.
- Actively engage in clinical supervision and mentorship.
- Develop formal mechanisms to share good practice, recent developments and research-based care.
- At all times keep the safety of the vulnerable adult in mind, and work within local safeguarding policies and procedures.
- To maintain working knowledge of legislation, policy, and guidance pertaining to learning disabilities and mental health.
- Maintain up-to-date and accurate case notes in line with professional standards.
- Maintain confidentiality.
Leadership
- Establish clinical credibility to influence professional and organisational boundaries, acting as a role model for clinical excellence.
- Encourage and support innovation, promote networking and explore new ways of working and sharing of expertise.
- Utilise clinical skills to ensure service users and carers receive the best possible care and outcomes, through clinically effective practice across the Learning Disabilities nursing service.
- Mentorship of unqualified staff and students.
Management Responsibilities
- To help oversee and manage support staff undertake their role, working to agreed evidence-based practice.
- Support others to make considered judgements relating to safeguarding concerns, utilising the skills and expertise of the Nurse Specialists, Safeguarding Children/Adults, where appropriate.
- Supports other health staff within the team in a professional and supportive manner in line with relevant policies.
- Acts responsibly to deal with difficult and complex situations and has the ability to assess effectiveness of actions and resulting consequences.
- Maintains a Personal and Professional Developmental portfolio.
- Accesses clinical supervision in line with policy. There may also be a requirement to provide clinical supervision to support staff if necessary.
- Demonstrates effective management and prioritising of own workload and diary management.
- Understand the principles of assessment and teaching in practice
- Evidence of Continuing professional development
- Experience in mentorship and supervisory roles
- Understanding of relevant policy, legislation drivers and application to clinical/service area e.g. Mental Capacity Act, STOMP, Transforming Care
- Demonstrates the use of evidence based practice
- Understanding of health related issues experienced by people with a learning disability
- Works in partnership with individuals, families, carers and other professionals
- A good understanding of Mental Health and the impacts on a persons wellbeing
- Good working knowledge/experience with Positive Behaviour Support
- Knowledge of Autism Spectrum Disorder (ASD)
- Ability to plan and implement work projects
- Evidence of leadership and motivational skills
- Experience of multi-professional partnership working across health, local authority and voluntary sector.
- Evidence of Safeguarding knowledge and experience with regard to children and vulnerable adults
