Key Responsibilities Conduct Care Act assessments, support planning, and risk assessments. Commission services and plan safe, person-centred discharges. Work creatively with adults to promote independence, wellbeing, and strengths-based practice. Collaborate with external agencies in a hybrid working environment (part office, part home-based). Essential Requirements Registered with Social Work England. In-depth knowledge of the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983. Effective communication, negotiation, organisational, and IT skills. Ability to work autonomously and manage complex casework. Commitment to interprofessional collaboration.
