If you believe that there are ways to drastically improve the way primary care is delivered for both patients and staff alike and you want to be a part of this; then we would love to meet you and tell you more about our plans.Job responsibilitiesKey ResponsibilitiesUndertake triage and clinical assessment of patients, including urgent, same-day, and routine presentations.Provide autonomous, evidence-based clinical care and treatment for patients, including prescribing medications in line with national/local guidelines and care pathways.Support home visits and provide an alternative model for GP urgent home assessments.Perform and interpret ECGs, blood pressure readings, spirometry (where trained), and other diagnostic tools.Undertake investigatory procedures and collect specimens including blood, swabs, urine samples, etc., within scope of practice.Request and interpret appropriate investigations to support clinical decision-making.Independently prescribe medications in accordance with local formulary, guidelines, and own scope of competence.Provide medication reviews and advice to patients on medicine use, interactions, and lifestyle considerations.Offer lifestyle advice and promote self-care where appropriate, including signposting to social prescribing and community services.Participate in delivery of health checks, chronic disease reviews, and anticipatory care planning (e.g., for frail or elderly patients).Work collaboratively with the MDT, including GPs, nurses, clinical pharmacists, care coordinators, and other allied health professionals.Attend and contribute to team meetings, case reviews, and safeguarding discussions.Communicate effectively with patients, carers, and external agencies, including handling sensitive or complex information.Maintain accurate, contemporaneous, and comprehensive clinical records.Ensure completion of necessary documentation related to consultations, referrals, investigations, and care plans.Adhere to information governance, safeguarding, health and safety, and confidentiality policies.Qualifications and requirementsDegree in Paramedicine or equivalent qualificationRegistered with the Health and Care Professions Council (HCPC) as a paramedicEvidence of ongoing professional developmentWorking towards or attained Level 7 (MSc) capability in clinical practiceExperience working in a primary care, urgent care, or general practice settingKnowledge and understanding of confidentiality, safeguarding, health and safety, and information governance policiesBenefits and leave27 days annual leave (pro-rata) plus 8 bank holidays (pro-rata)Enhanced family leaveDisclosure and background checkThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.Depending on experienceUp to £64,907, depending on experience #J-18808-Ljbffr
