School Nurse SEND senior Nursery Nurse

The Nursery Nurse will be part of skill mixed team of qualified SEND School Nurses, SEND Health Visitors, Staff Nurses, Nursery Nurse, administrative staff. The post holder as an effective member of the SEND 0-19 service will support the delivery of the Healthy Child Programme (HCP) 5-19 to a defined population. Using evidence based and needs led service; the post holder will offer packages of care in a variety of community settings, which include child health clinic, home, early year's settings, schools and children's centres. Main duties of the job The HCP 0-19 SEND team service is part of the wider Children's Health services which provides Community based services for children and young people (aged 0-19 years). The SEND School Nursing service element works with stakeholders across a number of settings and organisations to lead delivery of the Healthy Child Programme (5-19) to improve child health outcomes and ensure that families at risk are identified at the earliest opportunity. The team will work with a wider multi-disciplinary team and external services to provide a team around the child & family approach to service provision, ensuring an all year around service is offered. The overarching aim of public health services for children (0-19) is to protect and promote the health and wellbeing of children and families. The SEND is a workforce of specialist community public health nurses who provide expert advice, support and interventions to families with children and Young People between 0-19 years, and help empower young people to make decisions that affect their future health and wellbeing. Assist in the planning, delivery and evaluation of programmes of care which aim to improve physical and emotional health and wellbeing such as obesity/healthy eating, parenting programmes, childrens emotional health groups, health education & promotion activities. These programmes might be delivered to individuals or in groups. Support qualified nursing staff with the assessment of children and young peoples needs through individual health assessments, supporting assessment of emotional health needs, monitoring of growth and obtaining the voice of the child Provide care and support to identified families in a range of settings such as schools, clinics, their homes or community locations. Work in partnership with parents/carers, families and multi-disciplinary colleagues to improve the health and wellbeing of children and young people. Safely maintain confidential, professional records which are accurate, relevant, up to date and legible. E.g. Child Protection/Early Help Assessment. Create, maintain and enhance effective working relationships within the team, with other staff, and other agencies. Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with information governance policy The post holder must participate in clinical and other audits as required Participate in safeguarding supervision as per supervision framework. Person Specification Qualifications NCFE CACHE Level 3 Diploma in Child Care and Education/ BTEC National Diploma in Children's Care, Learning and Development/NVQ Level 3 in Children's Care, Learning and Development Foundation Degree or evidence post qualification education and/or development Experience of working with a Healthy Child Programme (e.g. School Nursing/health visiting) or community settings Evidence of working with CYP who have SEND and additional needs Delivery of health promotion sessions Demonstrable experience working with children and young people Can demonstrate high level of interpersonal skills. Computer literate - ability to use software programmes designed to maximise the contribution to the post e.g. Microsoft, Word, Excel.
