Our integrated service provides stroke rehabilitation and community neurorehabilitation support to service users with complex needs. As a Stroke Nurse Specialist, you will have the opportunity to work alongside a multidisciplinary team of highly skilled & experienced professionals, including Physiotherapists, Occupational Therapists, Speech and Language Therapists, Psychologists, Neuro Rehab Assistants, and other healthcare professionals. You will be responsible for managing a clinical caseload across Cambridgeshire & playing a key role in shaping the service, as well as supporting the development of your colleagues. You will carry out comprehensive nursing assessments and deliver evidence-based interventions. You'll work closely with the wider Multidisciplinary Team to ensure coordinated, patient-centred care. You should feel confident working across different settings, including patients' homes, inpatient wards, or community gyms, and be comfortable using virtual consultations to support your caseload. A smartphone and laptop will be provided to assist you in your role. Main duties of the job Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria. Provide high-quality specialist stroke advice & rehabilitation to stroke patients, supporting their independence and recovery. Manage a caseload of complex stroke patients, ensuring effective case management. Offer specialist assessments and guidance on diagnosis, medications, investigations, recovery, prevention, and rehabilitation. Advocate for patients, ensuring they receive necessary care and treatment. Support and train colleagues in conducting six-month reviews and contribute to ongoing training initiatives. Maintain compliance with local and national clinical standards (RCP & NICE). Utilise outcome measures and service data to demonstrate clinical effectiveness. Ensure high standards of governance, clinical safety, and patient experience. Identify and mitigate risks within the service. Provide individualised patient‑centred information on stroke diagnosis, investigations, and treatments. Offer secondary prevention advice and support interdisciplinary rehabilitation planning. Work collaboratively with acute stroke services, primary care, and community colleagues. Manage complex cases involving comorbidities and liaise with relevant professionals for optimal patient outcomes. Deliver emotional support and conduct mood assessments (e.g., PHQ‑9, GAD‑7). Identify rehabilitation goals using a multidisciplinary approach. Provide guidance on neuropathic pain, vision assessment, spasticity, and cognitive effects of stroke. Provide continence initial assessment & advice post stroke and make specialist referrals where necessary. Work across various settings, including patient homes, inpatient rehabilitation beds, care homes, and community locations. Person Specification Education / Qualifications Degree or equivalent e.g. Diploma entry level qualification into the Nursing profession. Current NMC Registration Professional knowledge and training that is stroke/neurological specific. Documented evidence of own continuing professional development Willingness to undertake Fieldwork/Practice Educator qualification. Member RCN Experience Relevant post‑graduate clinical experience in Stroke & rehabilitation Clinical experience working in stroke (acute/ rehabilitation) Clinical Supervisor Proven record of NHS experience in a variety of settings Involvement in regional or national clinical bodies e.g. clinical interest groups, conferences. Experience of being involved in research study. Experience of developing & delivering training to colleagues Skills & Abilities Ability to review clinical pathways and implement change. Able to demonstrate commitment to high quality care and service provision. Ability to work flexibly as part of a team. Advanced communication skills and report writing using standard keyboard skills using packages such as word. Able to assess plan and implement care. Good time management and prioritisation skills Able to impart skills/knowledge to others within formal/informal settings. Support junior staff and non‑registered staff Accepts responsibility and accountability for own work and can define the responsibilities of others. Recognises the limits of own knowledge & ability within the role and seeks and uses professional support appropriately. Understands the principle of confidentiality Knowledge of relevant outcome measures Knowledge in stroke rehabilitation /neurology SystmOne Awareness of equipment providers e.g.: NRS. Evidence of presenting at a local or regional level or willingness to develop skills. Physical Requirements The ability to kneel, bend & stoop, and work in cramped environments. The ability to use moving and handling equipment to manoeuvre patients.
