You will be proactive, able to make decisions and liaise with family and professionals when required. Experience supporting people with complex care needs, youth transition and supporting young adults who are care leavers. Ability to support people to work towards person-centred outcomes, enabling them to develop independence and maintain safety in their home and community. You should have experience in Autism, Learning Disabilities, ADHD, Epilepsy, and medication management and administration. About the Service Brackley House is a beautiful, detached property in the Heaton Chapel area of Stockport. It is a five‑bedroom property where 3 young gentlemen with complex care needs are currently living. The people we support are all aged between 19 and 24 years. Brackley House is a supported living project for 4 young men, when at capacity, with complex care needs. All the people supported are young men and some are care leavers who have transitioned into their own tenancies for the first time. The young men need support with daily activities: prompting and assisting with personal care needs, housekeeping, meals and cooking assistance, and taking part in local activities and accessing the community. It is a lively and fun service. A busy environment generating loads of smiles on a daily basis Shift patterns Day shifts – 8am to 8pm Waking Nights – 8pm to 8am We have a team on duty 24/7, so there are both day shifts and waking night shifts in place, and we provide additional 2‑1 hours of support for community-based activities.
