We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.The shift pattern for this post are Mon-Sun various hours between 8am and 8pm, weekend rota and public holiday coverPlease be advised that, effective 1 April 2026, the full-time working hours for all Agenda for Change posts will be reduced to 36 hours.The post holder will provide an accessible, appropriate, high quality, culturally sensitive and effective nursing service to all age groups in the community, by working in partnership with service users, care providers and other agencies.The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience to support and enable the district nursing team to provide a quality nursing service. The post holder will be a registered nurse with specialist district nursing qualification who supports in delivering care to patients within the caseload in hours and Out of Hours period.The post holder will be responsible for providing skilled nursing interventions for patients within the caseload. The post holder will coordinate workload and delegate work to other staff, and provide support to other members of district nursing team.The District Nursing service operates an open access policy that is available 365-days a year. The service does not operate a waiting list; however, care needs are prioritised to be able to respond to both planned and unplanned care. This includes the ability to respond timeously to unscheduled care requests for those patients who require urgent district nursing intervention.The post holder will participate in the back shift, weekend and public holiday rota. A current driving license essential.
