Physiotherapist – Discharge to Assess The closing date is 17 December 2025. The post involves working within the discharge to assess team, delivering holistic assessments and interventions to patients in their own homes or place of residence. Independent travel within the local geographical area is essential. Service Hours Mon‑Fri 08:00‑20:00 Sat‑Sun 08:00‑16:00 Responsibilities Manage a clinical caseload of patients with low to highly complex needs. Plan, implement, evaluate and modify individualised treatment programmes in patients’ homes. Supervise Band 3 support staff in caseload management. Work independently as a lone worker, liaising with community therapy teams, GPs, specialist agencies, social services and voluntary agencies. Ensure timely triage, discharge from acute settings and following appropriate pathways of care. Provide complex professional and clinical advice to patients, their families, carers and other health and social care professionals. Collaborate with colleagues to promote patient choice, participation and quality of life. Qualifications Professional registration with the HCPC. Honours degree or recognised qualification in Physiotherapy. Evidence of continuing professional development, including postgraduate courses and conferences. Membership of relevant special interest groups. Strong IT skills. Experience Broad range of general rehabilitation conditions. Evidence of managing complex caseloads; experience at Band 6 level. Previous community physiotherapy experience. UK driving licence or vehicle access. Previous involvement in audit, research and supervision.
