Job Title: Social Worker – Rapid Discharge to AssessLocation: Trafford (Hybrid)Pay Rate: £30 per hourJob Description: Liquid Personnel is recruiting an Adults Social Worker to join its client’s Rapid Discharge to Assess Team based in Trafford. What will your responsibilities be? Timely Hospital Discharge Planning: Work closely with health professionals to assess patients’ needs and coordinate safe, efficient discharge plans that avoid unnecessary hospital stays. Holistic Needs Assessment: Conduct strength-based assessments in community settings post-discharge to determine ongoing care and support requirements, ensuring person-centred outcomes. Multi-Agency Collaboration: Liaise with families, care providers, and partner agencies to implement tailored support packages and facilitate smooth transitions from hospital to home or interim care.
