If you want to be part of a forward thinking and innovative service, this is the opportunity you have been looking for.Key ResponsibilitiesWork closely with Occupational Therapists, Assistant practitioners, support staff, clinical assessors (UCR team), district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.Provide clinical triage, rehabilitation, falls management and support timely discharges from acute Trust partners.Visit patients within their own homes using a goal-oriented focus to maximise independence, safety and quality of life.Coordinate with health and social care colleagues to promote recovery and enable health, wellbeing and self-care.Manage your own caseload in the community and work closely with the MDT.QualificationsDegree/diploma in Physiotherapy and HCPC registered.Significant experience of working in NHS as a Physiotherapist.Excellent interpersonal, time management and communication skills.Ability to prioritise and work well as a member of a team.Full UK driving licence and vehicle for business use essential.BenefitsPaid enhancements for weekends and bank holidays.Annual DBS Update Service fee £16 per year.Induction period and supernumerary support given.Employee-led staff networks and inclusion initiatives.Commitment to reducing carbon emissions and achieving net-zero commitment.Applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment.
