About the Role Health and Social Care Officers (HSCOs) are responsible for triaging, co-ordinating referrals and providing information and advice to citizens and professionals at our busy 'front door' for Nottingham City Adult Social Care. You will enable decision-making and risk management on behalf of citizens requesting access to social care support. HSCOs play a key role in promoting the independence and well-being of the citizens referred in by providing advice, support and information. This then helps citizens navigate through to the right services based on the information you have gathered in this triage process. HSCOs assist in the delivery of an efficient and effective referral service for vulnerable adults, sometimes with complex needs or in difficult and unpredictable situations. Applicants should have a good understanding of the variety of services that are available to meet an individual's needs whether through preventative support or commissioned services. What skills are we looking for? Excellent customer service skills Highly developed organisational, administrative and negotiating skills A thorough understanding of adult social care issues To be diplomatic and approachable with the ability to develop positive relationships and have an awareness of the barriers that may affect communication. The ability to make judgements regarding referrals and requests in relation to complex adult social care situations and safeguarding concerns. You will need to have an understanding of risk management when triaging and coordinating referrals to make sure that citizens are seen by the most appropriate team/ service at the right time to meet their needs. The ability to handle telephone calls from distressed referrers, citizens or carers experiencing potentially complex problems and be able to act appropriately in a supportive, empathetic and courteous manner. -A DBS enhanced check for a regulated activity is required for this post.
