Practice Nurse

We are looking for 1 or 2 days a week Thursday 08:00 - 18:30 and Friday 08:00 - 14:00, you will be required to cover holidays and sickness The purpose of the post is to deliver safe, high-quality primary care nursing services to the practice population. Working within the NMC Code of Conduct the post holder will take a nurse role within the nurse's team, using previous primary care experience to provide nurse interventions To be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients. The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure. Main duties of the job Carry out nursing interventions and measurements for patients using clinical judgment. Administer medications by injection with the appropriate legal authorisation. Recognise health promotion opportunities and give brief, focused lifestyle advice using motivational interviewing techniques which may include brief intervention sign-posting. Undertake an initial assessment of patients presenting with wounds, produce treatment plans, and refer to other services as necessary. Remove wound closure materials including sutures, clips and staples. Take samples for the cervical screening program Administer vaccines to adults and children according to the UK immunisation schedule. Administer prescribed therapies such as contraceptive injections, vitamin B12, and depo neuroleptic medications recognising side effects and contraindications. Sexual health Advise women on suitability of oral and long term contraceptive methods and be aware of local referral pathways. Long term conditions Provide nurse-run asthma/diabetes/COPD service to patients t, including screening, diagnosis, ongoing management, and referral to other agencies in line with NICE guidance and Quality and Outcome Framework indicators.
