We are looking for 1 or 2 days a week Thursday 08:00 - 18:30 and Friday 08:00 - 14:00, you will be required to cover holidays and sickness The purpose of the post is to deliver safe, high-quality primary care nursing services to the practice population. Working within the NMC Code of Conduct the post holder will take a nurse role within the nurse's team, using previous primary care experience to provide nurse interventions To be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients. The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure. Main duties of the job Carry out nursing interventions and measurements for patients using clinical judgment. Administer medications by injection with the appropriate legal authorisation. Recognise health promotion opportunities and give brief, focused lifestyle advice using motivational interviewing techniques which may include brief intervention sign-posting. Undertake an initial assessment of patients presenting with wounds, produce treatment plans, and refer to other services as necessary. Remove wound closure materials including sutures, clips and staples. Take samples for the cervical screening program Administer vaccines to adults and children according to the UK immunisation schedule. Administer prescribed therapies such as contraceptive injections, vitamin B12, and depo neuroleptic medications recognising side effects and contraindications. Sexual health Advise women on suitability of oral and long term contraceptive methods and be aware of local referral pathways. Long term conditions Provide nurse-run asthma/diabetes/COPD service to patients t, including screening, diagnosis, ongoing management, and referral to other agencies in line with NICE guidance and Quality and Outcome Framework indicators. About us We are a friendly Dispensing Practice in a Village on the outskirts of Rugby and Coventry with a patient list of approximately 4,500 patients Training Practice with 2 Partners and 1 GP Retainer Dedicated nursing team working with HCA to deliver chronic disease management Dedicated Reception/Dispensing team with 6 Receptionists/Dispensers Job responsibilities Primary Key Responsibilities Thefollowing are the core responsibilities of the Practice Nurse. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and staffing levels: Toact a or support the Infection Prevention Control (IPC) lead and to work to theIPC Policy providing the implementation of audits, training, support andevidence of compliance to the practice manager Toproduce or support the production of the IPC annual statement in conjunctionwith the practice manager Todevelop, implement and embed an effective care management programme for thefrail and housebound patients registered at the practice Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice Todevelop, implement and embed health promotion and well-being programmes Toimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeing Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health Asrequired, to provide routine nursing care to patients in accordance withclinical-based evidence, NICE and the NSF Asrequired, to diagnose and manage acute and chronic conditions Toprescribe medication for patients in accordance with PGDs and own scope ofpractice Toreview patient medications to enhance compliance Toco-ordinate the provision of travel medicine services Torequest pathology services and process pathology results as required Tooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required Tomaintain accurate clinical records in conjunction with extant legislation Toensure read codes are used effectively by all members of the nursing team Toensure the appropriate maintenance and use of chronic disease registers Todevelop, implement, embed and participate in well woman clinics Todevelop, implement, embed and participate in well man clinics Toensure compliance with the practice chaperone policy Toassist GPs with minor surgery when required Toprioritise health issues and intervene appropriately Tosupport the team in dealing with clinical emergencies Torecognise, assess and refer patients presenting with mental health needs Toimplement vaccination programmes for adults and children Tosupport patients in the use of their prescribed medicines or over-the-countermedicines (within own scope of practice) Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.) To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual) To support the clinical team with allsafeguarding matters, in accordance with local and national policies To understand practice and localpolicies for substance abuse and addictive behaviour, referring patientsappropriately Todeliver opportunistic health promotion where appropriate Secondary Responsibilities Developand review audit protocols and process for the practice, effectively utilisingthe audit cycle Develop,review and implement nursing protocols in conjunction with the partners Supportall members of the nursing team, providing guidance when necessary, conductingappraisals and training needs analyses as required Participatein local initiatives to enhance service delivery and patient care Supportand participate in shared learning within the practice in order to improvepatient care Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate Participatein the review of significant and near-miss events applying a structuredapproach, i.e., root cause analysis (RCA) Coordinatethe nursing team rota, ensuring sufficient staff are available to meet patientneeds Leadthe planning and implementation of changes within the nursing team, enhancingthe provision of services to patients Support the practice team with thereviewing and implementation of practice policies and protocols, ensuringconformance to extant legislation The job description and person specification may be amended followingconsultation with the post holder to facilitate the development of the role,the organisation and the individual.
