All Age Continuing Healthcare Clinical Commissioning Case Manager

Job Summary Are you a qualified RGN, RMN, or RNLD looking for a rewarding role with professional autonomy and the chance to make a real difference? We're recruiting a Clinical Commissioning Case Manager to join our Continuing Healthcare team across Birmingham and Solihull. You will manage a caseload of individuals eligible for NHS Continuing Healthcare, ensuring their care is safe, appropriate, and effectively commissioned. Key Responsibilities Coordinate care for individuals with complex needs. Work independently and within a multidisciplinary team. Commission care packages and liaise with families and providers. Apply clinical judgement to assess needs and develop care plans. Ensure compliance with national frameworks and local policies. Provide high‑quality resources, professional advice, guidance and expertise to the ICB and partner agencies. Autonomously manage a caseload of individuals with highly complex health needs, providing significant clinical guidance to providers. Attend multi‑disciplinary meetings, case conferences and discharge meetings to ensure continuity of care across agencies. Negotiate care package costs and monitor fully funded care packages. Ensure care packages are reviewed within agreed time scales and recorded in the ICB database. Identify and process safeguarding and quality issues, ensuring welfare is protected. Apply the Mental Capacity Act, sourcing best interests and DOLs when required. Support the Head of CHC and Lead Nurses in ensuring appropriate individual placements provide safe, high‑quality care. Deputise for the Clinical Services Delivery Manager as required. Contribute to and deliver training on CHC and FNC to a variety of audiences, particularly in health and social care. Qualifications Registered Nurse (RGN, RMN, or RNLD) with relevant experience. Strong caseload management and prioritisation skills. Excellent communication and interpersonal abilities. Interest or experience in commissioning and complex care. Person Specification Essential Educated to degree level or equivalent level of experience working in a specialist area. Professional qualification and current registration with relevant professional body (e.g. NMC). Previous experience of undertaking all age Continuing Care Assessments. Experience of negotiating and liaising with multi‑disciplinary team. Experience of complex case management. Knowledge of MCA, DOLS, LPS. Good understanding of safeguarding issues and policy framework. Good understanding of the CHC National Framework. Desirable Experience of commissioning. Experience of line management of staff.
