Job Summary The Hospital at Home Frailty Virtual Ward provides acute treatment and monitoring in patients' homes to prevent hospital admissions and deterioration. It also delivers Urgent Community Response, offering rapid assessment and treatment for frail or complex patients. This is a Band 5-6 developmental post with a bespoke training and competency programme designed to support progression from Band 5 to Band 6. The post holder will follow a structured development plan with clearly defined competencies, learning objectives, and milestones. They will be supported by an assigned mentor and practice supervisor, enabling them to develop the clinical knowledge, skills, and confidence required to work autonomously within the Hospital at Home Virtual Ward. The post holder will provide holistic assessment to support management in the community through face‑to‑face assessments. Responsibilities Supporting admission avoidance by delivering treatment and care planning after an initial assessment by senior team members. Collaborating with the Multidisciplinary Team (MDT) to provide person‑centred care. Delivering interventions at home to include but not limited to holistic frailty reviews placing the person at the centre of care, re‑assessing and monitoring treatment plans, point of care testing, IV fluids, IV antibiotics, ECGs. Delegating tasks and supervising junior staff. Working on the Falls Car, alongside SCAS, to assist uninjured fallers and ensure safe recovery at home with a full holistic and functional review. Main duties of the job Work within professional codes and maintain compliance with regulatory standards. Demonstrate flexibility in a dynamic environment and promote evidence‑based practice. Provide holistic assessments and patient‑centred care without direct supervision, using critical thinking and recognising limitations. Deliver interventions such as IV therapy, ECGs, and care planning for acute and chronic conditions, collaborating with senior clinicians and the MDT. Manage caseloads, ensure accurate record‑keeping, and maintain confidentiality. Support self‑care, supervise junior staff, and act as a role model for person‑centred practice. Participate in team meetings, performance reviews, and service improvement initiatives. Ensure safe clinical practice within health and safety guidelines. Delegate tasks effectively, advocate for patients, and contribute to compliance with CQC and contractual standards.
