Home Visiting Nurse/Paramedic – Hospital at Home team Closing date: 18 December 2025 Job summary This dual role works across two complementary services – a GP home visiting service and the Hospital at Home (H@H) Frailty Virtual Ward. The post holder delivers high‑quality care in patients’ homes or care homes, supporting short‑term acute needs and longer‑term interventions that help patients remain safely out of hospital. Working independently you provide face‑to‑face assessments, make safe and effective clinical decisions, and provide expert care within your professional boundaries and scope of practice. Main duties of the job Clinical Assessment & Diagnosis: Autonomously perform comprehensive history, physical examination and clinical assessment in patients’ homes or care homes; diagnose and manage undifferentiated, acute and complex conditions; undertake and interpret diagnostic tests (e.g. ECG, urinalysis, blood tests, point‑of‑care testing) to inform decision‑making. Treatment & Care Planning: Provide urgent and routine care, including prescribing or recommending appropriate treatment plans in collaboration with the GP or clinical team; develop and implement management and treatment plans for patients with acute illness, exacerbations of long‑term conditions or frailty‑related deterioration. Home Visiting, Hospital at Home & UCR: Respond to same‑day visit requests for acute illness, minor injuries or worsening of chronic conditions; conduct home visits for patients unable to attend the practice; deliver care as part of the Hospital at Home (H@H) Frailty Virtual Ward, providing treatment including IV therapy, monitoring and review at home for patients who would otherwise require admission.
