Job Summary An exciting opportunity has become available within the COPD Inreach Nursing Team for a part time nurse or physiotherapist. This role would suit a current Band 6 who wants to specialise in Respiratory or an experienced Band 5 who wants to progress. About the Team The Inreach COPD Specialist Nurse team aim to review all patients admitted to hospital with an exacerbation of COPD within 24 hours of admission, complete medication and inhaler technique reviews, review and provide self‑management plans, discuss pulmonary rehabilitation and complete a discharge bundle. You will also be involved in providing post‑discharge phone calls to patients, completing NRAP COPD Audit, and working closely with the COPD Virtual Ward and community COPD Team, specialist respiratory nurses at Worcester Royal and Alexandra Hospital, and the Home Oxygen Team. Key Duties Provision of Specialist Clinical Services The post‑holder will: 1.1 Act as a specialist, autonomous clinician for patients with COPD both in hospital and in community settings. 1.2 Provide care to patients with chronic respiratory disease as outlined in the COPD Team standard operating procedures (SOPs) and associated team and trust policies. 1.3 Take a role in the assessment and treatment of COPD patients, determining a clinical diagnosis, treatment and management plan through comprehensive clinical reasoning. 1.4 Perform and interpret specialist assessments of patients with COPD in acute and non‑acute situations, including comprehensive history, physical examination, lung function testing, blood gas sampling, exercise testing, functional assessment and psychosocial status. 1.5 Manage COPD patients in their own home during an acute exacerbation using specialist respiratory techniques such as oxygen, nebulised medication, NIV and sputum management, while providing monitoring, education and referrals. 1.6 Evaluate the needs of individual patients and plan, deliver and evaluate all elements of COPD management in various settings. 1.7 Contribute to respiratory clinics where appropriate, both in acute and community settings. 1.8 Be responsible for the ongoing management of a caseload of patients with chronic respiratory disease, organising the caseload effectively and efficiently with regard to clinical priorities and time management. 1.9 Construct specialist self‑management plans for patients and assess and monitor progress, identifying barriers and minimising their impact on optimal care. 1.10 Communicate effectively with patients and carers to maximise rehabilitation potential and ensure understanding of the condition to enable effective self‑management, using all available resources to gain informed consent. 1.11 Assess capacity and work within a legal framework with patients who lack the capacity to consent. 1.12 Communicate complex patient‑related information effectively with MDT members across health & social services, private providers and other agencies to ensure the delivery of a co‑ordinated multidisciplinary service. 1.13 Participate in MDT management of respiratory patients ensuring a best‑practice approach. 1.14 Provide expert clinical education and support to medical staff of all levels, nursing and therapy staff, patients and carers, and students. 1.15 Contribute to the development of specialist protocols and care pathways for people requiring a respiratory service, working closely with key colleagues and partners from health and social care. 1.16 Participate in 6‑day‑per‑week cover of the service (Mon‑Sat). Education and Training 2.1 Participate in the Trust personal development review (PDR). 2.2 Agree service and personal objectives with the Clinical Team Lead and be responsible for self‑audit to demonstrate achievement of objectives and evidence of value to the trust. 2.3 Provide specialist advice, teaching and training of other healthcare professionals on optimal management of patients with COPD on a formal and informal basis. 2.4 Advise, educate and train other members of the COPD team to ensure high‑quality clinical care. 2.5 Commit to a philosophy of care emphasising self‑management and patient empowerment through health education and promotion activities. 2.6 Be an active member of the in‑service training programme, attending and delivering presentations at staff meetings, tutorials and training sessions internally, and attending external courses. 2.7 Actively participate in research projects where appropriate, reporting findings within a multidisciplinary framework to facilitate service evaluation and quality improvement. 2.8 Act as a clinical educator for students within the team, providing appropriate supervision, education and assessment. Professional 3.1 Be professionally and legally accountable for all aspects of own work and caseload management, supporting junior staff to achieve high standards. 3.2 Maintain a high level of health and safety, including knowledge of the lone‑working policy. 3.3 Demonstrate a working knowledge of clinical governance and risk management strategies relevant to the role. 3.4 Comply with Health and Safety policies and report incidents via the appropriate channel (e.g. Datix). 3.5 Maintain continuing professional development records and remain fit to practice. 3.6 Participate in the COPD Team strategy group in planning and organising delivery, standard and modernisation of the Community COPD Team services. 3.7 Plan, organise and conduct own areas for practice‑based research, audit and service evaluation with the Clinical Team Lead. 3.8 Ensure the service is developed as a needs‑led service with consideration of demographics and partner service developments. 3.9 Work with local GPs, ambulance service and Community Health to promote and develop the service in the community. General Duties 1. Be responsible for all aspects of the line management of junior practitioners. 2. Assist the Clinical Team Lead with recruitment if appropriate. 3. Advise, support and counsel junior team members on personnel and professional matters. 4. Use trust clinical software (e.g. Bluespier, MS Word/Excel/PowerPoint) to produce documents and support delivery of key roles and responsibilities. 5. Understand the financial position of the service. Person Specification Personal Qualities (Essential) Demonstrates a commitment to personal and team continued professional development. Willing to change, accept change and explore new ways of doing things. Qualifications (Essential) Currently Registered Nurse with the relevant UK professional body (NMC). Meets the standards for continuing professional development as outlined by the relevant professional body. Desirable Relevant validated clinical course. Evidence of relevant clinical specialist knowledge in respiratory care acquired through degree‑level training, CPD or portfolio. Evidence of coaching and supporting learners in practice (practice‑based assessor &or supervisor). Experience (Essential) Demonstrable hospital and/or community experience delivering clinical care to general and specialist areas, including respiratory medicine, as a Band 5 or above. Experience of working effectively both within a multi‑disciplinary team and independently. Experience of educating patients in self‑management of their condition. Desirable Experience Experience of working in a community setting (e.g., community hospitals, patient home visits, community pharmacy). Awareness of the Pulmonary Rehabilitation Programme. Experience of audit and clinical governance within clinical practice. Other Job Requirements (Essential) Holds a current valid UK driving licence and can travel within the post requirements. Able to work weekends on a rota basis. Able to occasionally work outside of office hours to meet the demands of the post. Knowledge & Skills (Essential) Experience of physical assessment and clinical management of respiratory patients. Able to interpret national guidelines, Trust policies, procedures and guidelines related to respiratory medicine. Able to maintain judgement and work effectively under pressure.Able to maintain contemporaneous, accurate and up‑to‑date patient records. Computer literacy. Ability to act on own initiative and take responsibility for own caseload, demonstrating effective time management. Able to work alone and be aware of Health and Safety of lone working.
