Job Summary Modality Partnership Haydock Division is hiring two part‑time Call and Recall Care Coordinators (30 hrs per week and 27 hrs per week). The postholder will support patient care and divisional clinical targets by managing patient call and recall activities, including data collection, monitoring, tracking and reporting. The role requires innovation, flexibility and commitment, with working hours that are not fixed to 9 am‑5 pm. Key Responsibilities Coordinate and recall patients for chronic disease management. Book appointments with appropriate clinicians to ensure timely care delivery. Use active communication to manage patients and reduce DNAs. Perform administrative duties: data collection, monitoring, tracking and reporting. Collaborate with management and clinical teams to optimise clinical time utilisation. Qualifications & Experience Experience in primary care settings. Proficiency with MS Word, Outlook, Excel and related software. Knowledge of medical terminology and NHS systems is desirable. Understanding of data protection, patient confidentiality and safeguarding policies. Essential Skills Customer‑service orientation. Excellent listening, communication and interpersonal skills. Accurate typing and strong administrative and organisational skills. Ability to follow policies, practices and protocols. Computer‑literate and adaptable to diverse software. Good telephone manner and strong team‑player attitude. Responsive and positive outlook.
