Your Role We believe in providing care and support to clients, enabling them to make informed decisions that increase wellbeing, independence and life skills. You will work within supported services to deliver holistic support across a range of settings. Key Responsibilities Carry out assessment to identify and prioritise needs. Use SMART goal planning to provide needs‑led holistic support. Create an environment that promotes opportunities for customers to develop, learn and gain skills toward independence. Provide comprehensive support planning in line with department policies and procedures. Ensure customers are fully informed of their rights and responsibilities and are empowered to participate in service running. Set up and maintain customer files and ensure accurate and up‑to‑date records in accordance with HCS policies. Lead on co‑producing bespoke support and move‑on plans, organising regular planned reviews of support and risk plans. Engage customers to meet agreed outcomes, develop life skills, assist with day‑to‑day support and tenancy matters, and promote employment, education and training opportunities. Signposting customers to appropriate external services, support financial independence through budgeting, and manage health needs. Empower customers to self‑manage medication and lead support initiatives, including group work. Ensure customer safety by following safeguarding procedures and escalating risks appropriately. Record and update clear, factual, strengths‑based information on the local digital platform. Facilitate referral processes and assess potential new customers. Contribute to housing management service, including income collection and tenancy support. Maintain safe environments by reporting repairs and health & safety concerns; clean and prepare rooms as required. Resolve difficult situations sensitively and balance individual needs with service safety. Develop and maintain local partnerships for holistic support. Carry out day‑to‑day administration and operational duties. Work flexible hours and travel to different properties as needed; use Lone Worker system when necessary. Adhere to Riverside policies on health & safety, safeguarding, GDPR, cash handling, etc. Deliver role in line with Riverside company values – “Our Riverside Way”. Participate in team meetings, regular supervision, and reflective practice. Undertake training and continuous development; accept additional duties as required. Essential Qualifications Understanding of barriers faced by vulnerable and diverse customer groups. Experience delivering structured support and risk management. Team player with caring, empathetic, flexible, and resilient attitude. Experience positively resolving incidents. Proactive initiative and confident decision‑making. Competent administrative and IT skills for reporting and communication. Desirable Qualifications Knowledge of psychological or trauma‑informed approaches. Experience in challenging environments with strong planning and prioritisation. Knowledge of current benefit systems. Experience in a care and support environment.
