This position will work alongside our Clinical Pharmacist and prescribing admin team to support our existing clinicians in providing a robust and safe prescribing process to our patients. Medication reviews, prescription queries, re-authorising of medications and adding/removing medications following specialist advice will be part of the day to day role.Main duties of the jobThe Pharmacy Technician's core responsibilities will cover clinical, technical, and administrative duties. Working alongside our clinicians and Pharmacist and supportive administrative team, the post holder will ensure effective and efficient use of medicines. This will involve targeted medication reviews, supporting patients and carers post-hospital discharge, health promotion, and support for self-care. Assistance will also be given to our current prescribing admin team with every day queries.About usNeasham Road Surgery has a registered list of 13,500 patients. Our team of 5 GP's and 4 Nurse Practitioners, along with our team of Practice Nurses, HCA's and admin staff provide good quality care whilst maintaining a friendly and supportive environment. We achieve a high QOF figure with a CQC rating of good. We can offer you support, learning, and a laugh Job responsibilitiesThe pharmacy technician will support the surgery by working withthe clinical team to deliver a programme of medicines optimisation andreconciliation from hospital discharge and other secondary care communications.This will enable safe discharge of patients from hospital to home/ care home,ensuring medication changes are safe and timely managed with patientcounselling where appropriate. The post holder will work within their owncompetency with support from the partners and other staff within the Practice. Clinical Supervision will also be availablefrom a Clinical Pharmacist within the PCN.The post holder will support the practice team with specificquality improvement projects in relation to medicines for example opioidreduction management.Key responsibilities:Undertake medicines reconciliation for patients discharged fromhospital to ensure new medicines are recorded accurately on the GP clinicalsystem.Update GP system of new medicines initiated by secondary careconsultants which are to be continued in primary care.Medication monitoring requirements are fulfilled including safemanagement of shared care drugs.Monitoring creatinine clearance for DOACs and checking correctdoses, which will include changing appropriate patients to edoxaban.Alert staff of additional safety checks required for newmedications.Community pharmacy to be informed of changes to mediboxprescriptions and repeat dispensing is updated.Assist with medication queries from GPs and other clinicians orpractice staff.Assist with delivering Quality Improvement Projects aimed atimproving medicines management, medicines safety and polypharmacy.Work with Community Pharmacies supplying care homes to sort outsupply issues for the monthly medication cycle and interim prescriptionrequests. Action requests from care staff for prescriptions.Update dose instructions for all medications where there are outof date titrating instructions, unclear or ambiguous instructions.
