Job Summary An exciting opportunity exists for a Clinical Nurse Specialist in Palliative Care to gain experience working in a community setting (1.0 WTE). The post holder will work predominantly in the community setting with flexibility to work within the whole of the Specialist Palliative Care service as is required. This is a 7 day service which runs from 08:00 to 20:00. Responsibilities Use advanced skills and expert knowledge to make comprehensive and focused assessments. Identify subtle changes in condition and act on these appropriately. Work in partnership with clinicians and other health professionals to plan and implement treatment for palliative care and patients with life limiting illness. Refer patients to other services or professionals as appropriate and facilitate discussion with the patient, their family and significant others concerning palliative care and end of life care plans. Provide specialist palliative care to patients with complex problems and act as a specialist resource to generic health and social care professionals. Deliver education and training to develop the palliative care practice of multidisciplinary team members. Work both independently and as part of a multidisciplinary team, communicating effectively with a wide range of healthcare professionals. Person Specification – Qualifications First level nursing registration. Current NMC registration. First level degree or undertaking a first level degree. Willingness to undertake further academic or practical study at a higher level e.g. Master’s degree. Teaching qualification or willingness to undertake. Advanced Communication Course (desirable). Independent nurse prescribing qualification (desirable). Specialist palliative care qualification (desirable). Person Specification – Experience Community experience with palliative care patients. Up to date clinical knowledge in palliative care. Considerable post registration experience. Experience working with national policies and reforms relevant to the specialty. Experience of teaching, multi‑agency working and implementing evidence‑based practice. Experience of cross boundary working in partnership with health, social and voluntary sectors (desirable). Experience of participation in research, audit and evaluation (desirable). Experience of using counselling skills (desirable). Person Specification – Skills & Knowledge Knowledge of NMC code of professional conduct. Ability to communicate complex and sensitive information with empathy and reassurance to patients, families and professionals. Knowledge of clinical governance and its implications. Can manage a caseload and demonstrate time‑management skills. Collaborates with the multidisciplinary team, negotiating and influencing others. Computer skills. Knowledge of research processes (desirable). Influencing, negotiating and counselling skills in highly emotive settings (desirable). Ability to challenge and be challenged regarding triage outcomes (desirable). Key Attributes Awareness of self as a role model. Full, valid UK driving licence and access to a car for business use (unless you have a disability as defined by the Equality Act 2010).
