Clinical Nurse Specialist/Paramedic Specialist Palliative Care Join to apply for the Clinical Nurse Specialist/Paramedic Specialist Palliative Care role at Central and North West London NHS Foundation Trust Job Overview We are recruiting a Band 7 Clinical Nurse Specialist / Clinical Paramedic Specialist Palliative Care for our community palliative care team in South Camden and Islington. This is a fixed‑term post to cover maternity leave. We are looking for someone who is resilient, motivated, and demonstrates compassion, self‑knowledge, confidence, generosity, and courage. The role is part of a thriving multi‑disciplinary team working daily to achieve the best outcomes for people we support, with a focus on quality of life and end‑of‑life decision‑making. In the community you will work as an autonomous practitioner, part of a team managing a caseload of palliative care patients with a variety of complex needs. The role includes providing on‑call cover for UCLH hospitals and working within a large team of nurses who provide comprehensive palliative care in diverse community settings. Main Duties of the Job Act as an expert clinician for patients and families with complex palliative needs, taking a key worker role where appropriate. Independently manage a caseload of patients with complex needs, including receiving and making referrals. Assess referrals from various sources, establish priorities, determine needs, and initiate treatment plans, supporting junior clinicians as needed. Provide palliative care advice within general multidisciplinary teams, even for patients not currently under palliative care. Deliver skilled clinical interventions addressing quality of life, prognosis, and end‑of‑life decision‑making within an ethical framework. Serve as an expert advisor on pain and symptom management, collaborating with professionals to enhance their palliative care skills. Independently assess and support bereavement needs of informal carers, making appropriate referrals. Work autonomously in diverse community settings, considering local socio‑economic and cultural factors. Lead responses in crisis situations, including family conflict, emergencies, and professional distress. Use in‑depth knowledge of multidisciplinary roles and external agencies to ensure optimal referral and care pathways. Working for Our Organisation The community team provides palliative care to patients of South Camden & Islington and UCLH. Based at St Pancras Hospital, we work as one team with the UCLH hospital team. Over 25 years the team has grown from one consultant nurse and a doctor to nearly 30 staff, including CNSs, Consultant Nurses, Doctors, OTs, social workers, physiotherapists, and administrative colleagues. Detailed Job Description and Main Responsibilities Be professionally and legally accountable for all work performed, adhering to the Standards for Better Health, Palliative Care Team guidelines, CNWL and UCLH NHS Trusts, NMC Code of Conduct/HCP policy. Undertake specialist clinical assessment of clients with complex palliative care needs through subjective questioning, objective examination, and review of relevant written material. Prioritise factors involved in patients and informal carers’ conditions using advanced clinical reasoning and current professional opinion. Identify patients, informal carers, or children who may be at risk and vulnerable; promptly enact the policies for Care of Vulnerable Adults and Children. Person Specification Experience – Essential Criteria Extensive post‑registration experience in the relevant specialty. Experience in case or ward management. Experience teaching clinical staff, patients, and families. Experience – Desirable Criteria Demonstrated ability to initiate, evaluate, and sustain change management. Experience caring for patients in their own homes. Qualifications – Essential Criteria Registered Nurse (RN) or Registered Paramedic. Relevant post‑basic qualifications. Recognised teaching qualification. Achieved or working towards a first degree. Qualifications – Desirable Criteria Non‑medical prescriber qualification. Physical assessment qualification. Skills and Knowledge – Essential Criteria Contemporary and relevant clinical knowledge and expertise. Ability to work independently and as part of a team. Basic computer literacy. Strong negotiation skills with clinical professionals, patients, and families. Effective workload prioritisation and time management. Problem‑solving skills and ability to take initiative. Experience delivering evidence‑based practice. Excellent verbal and written communication skills. Skills and Knowledge – Desirable Criteria Ability to supervise and develop staff. Experience working with integrated care pathways. Audit and Research – Desirable Criteria Experience in audit, standard setting, and quality improvement. Ability to incorporate research into practice and motivate others. Experience involving service users in planning and delivery. Professional Requirements – Essential Criteria Evidence of ongoing professional development. Maintenance of professional registration.
