The team works closely with a multidisciplinary network of health, social and third sector staff including Specialist Nurses, hospitals, hospices and GP surgeries. Job Details Date posted: 05 December 2025 Pay scheme: Agenda for change Band: Band 5 Salary: £31,049 to £37,796 a year per annum / pro rata Contract: Permanent Working pattern: Full-time, Flexible working Reference number: 150-MT2262-CC Job locations: Brighton General Hospital, Elm Grove, Brighton, BN2 3EW Responsibilities Deliver multi‑professional high quality care in the community, working with patients towards rehabilitation, self‑management and independence throughout the continuum of care. Participate in a team of colleagues supporting evidence‑based care and facilitating staff learning and development. Manage patients day‑to‑day under the supervision of Community Sisters, charge nurses and Clinical Lead. Provide wound care, continence management, palliative care (including syringe drivers), venepuncture, IV therapy and medication management. Conduct holistic assessment, care planning, individualised care and promotion of self‑care. Perform safe and effective review and referral to the appropriate member of the multidisciplinary team. Prioritise workload, delegate appropriately and support professional development. Maintain awareness of health, safety and security risks and engage in service provision and change activities. Work autonomously and as part of a team, using SystmOne mobile working. Qualifications & Experience Registered Nursing Part 1 (current registration with the Nursing and Midwifery Council). Good communication, negotiation, numeracy and literacy skills. Demonstrated competency in a range of clinical skills including wound care, palliative care, catheterisation, medicines management and IV administration. Knowledge of current local policies and the application of research‑based practice. IT literate or willing to achieve within an agreed period. Car driving licence and access to a car for work purposes. Desirable: 1 year post‑registration, experience of community nursing, experience using computerised patient notes systems, motivational interviewing skills, and ability to work as part of a multidisciplinary team.
