Vacancy at Northumbria Healthcare – NHCT Northumbria Healthcare NHS Foundation Trust An exciting opportunity has arisen within the MHSOP Community Mental Health Team for a 12‑month fixed‑term period to operate as a Community Mental Health Nurse within the Older Adult’s pathway within a specific geographical area. Key responsibilities include: Providing specialist skills, knowledge and experience for those identified as having mental health problems. Working as part of a community team, which may be integrated, in collaboration and partnership with other services. Being compassionate in meeting the needs of service users and their carers, and being professionally accountable for their care. Carrying out a range of clinical/therapeutic interventions within the service user’s home environment or other community settings. Performing comprehensive assessment, intervention, advice and support for service users, families, carers and other professionals. Ensuring high quality, person‑centred care, managing risks and offering appropriate (pharmacological and non‑pharmacological) interventions timely. Contributing to effective care planning, regular reviews, risk assessment and safe discharge planning. Role‑modeling compassionate and inclusive leadership to shape a collective leadership culture within the Trust. Working as a community mental health nurse, you will undertake the assessment and management of service users presenting with complex mental health care needs.
