Job OverviewAn opportunity has arisen for Community Mental Health Nurses (Care Co‑coordinator / Key Worker) looking to join a great team, providing specialist mental health care to the population of North Tameside. The Secondary Care CMHT supports residents with severe and enduring mental health conditions and aims to provide outstanding care to the people of Tameside.This is an exciting time for CMHT as it undergoes a transformation within our service as part of national and local developments. We provide high‑quality mental health and learning disability services, both inpatient and community, across five boroughs of Greater Manchester.Detailed Job Description and Main ResponsibilitiesUndertake timely, comprehensive, holistic mental health assessments using Trust‑approved documents, including mental health reviews and risk assessments, and formulate risk and well‑being care plans.Identify and implement effective risk management plans for patients under your care.Provide person‑centred well‑being care plans with SMART goals.Promote choice and service user involvement in care planning and risk management.Ensure practice aligns with social inclusion and recovery models, using a strengths‑based approach.Follow the nursing process (Assess, Plan, Implement, Evaluate).Maintain good knowledge of frequent psychotropic medications, including doses, side effects and routes of administration.Be competent in the administration of intramuscular injections and understand safe medication storage.Provide evidence‑based therapeutic interventions in accordance with NICE guidelines, appropriate to patient needs.Offer education, advice, emotional and psychological support to patients to promote re‑enablement and recovery.Demonstrate effective communication with patients, families, carers, the MDT and external stakeholders, maintaining confidentiality and information governance. Keep accurate, contemporaneous records using the PARIS system, adhering to Trust policies.Support and collaborate with colleagues, MDT members and outside agencies for the best interests of the service user.Develop strong working relationships with the MDT to promote improved outcomes for patient care.Apply knowledge of the MCA 2005 and other legislation to support assessment of capacity and provide training to others.Show understanding of MHA 1983, MCA 2005 and Care Act 2014.Provide leadership support to junior team members (Band 5 nurses, community support workers, student nurses) and contribute to team discussions.Demonstrate good teamwork, accountability, and share skills to support team development.Ensure high quality of care using the 6Cs of nursing, Trust values and policies.Recognise limitations and seek appropriate support when needed.Refer to the local authority for Care Act assessments and offer carers Carer Assessments.Identify, report and investigate safeguarding concerns.Complete Social Circumstances Reports for detained patients where detention is reviewed.Person SpecificationEducation / QualificationsRegistered Mental Health NurseCurrent registration with appropriate regulatory body (e.g., NMC)Completion of preceptorshipDesirableQualification in psychological therapeutic interventionsMentorship qualificationExperienceSubstantial post‑registration experience in relevant practiceExperience managing a caseloadExperience working in a multi‑disciplinary teamExperience working with people with severe and complex mental health problemsDesirable ExperienceExperience of clinical supervision of other staffExperience working within a community settingKnowledgeGood knowledge of therapeutic assessment techniques and behaviour management for complex needsKnowledge of screening and assessment tools used with severe and complex mental health problemsComprehensive understanding of safeguarding adults procedures and practiceComprehensive understanding of the Mental Capacity Act, Mental Health Act and Care ActKnowledge of roles and responsibilities under the Care Programme ApproachUnderstanding of key national and local policies and drivers in adult mental health servicesDesirable KnowledgeKnowledge of local authority and CCG commissioning processesSkills & AbilitiesAbility to undertake the care coordinator roleEvidence of ability to provide specialist mental health assessment, risk assessment/management and accept accountability for decision makingExcellent written and interpersonal communication skillsAbility to manage emotions and interact constructively in high‑stress situationsCapability to deliver therapeutic interventions for this client group and carersCompetence in preparing, recording and completing professional care records and reportsAbility to manage a diverse and complex caseload, prioritise time and work under pressureAbility to coordinate and delegate the work of junior staffAbility to value diversity and respond constructively to discriminatory behaviourIT skills #J-18808-Ljbffr
