Our goal is to provide housing with care based on an enabling approach with an outcome focused support planning process.As a large provider of social care services, Mears works collaboratively with multi-disciplinary teams to address challenges in the social care marketplace.We are Mears Supported Living Ltd, providing a range of care and support services to people out in the community and in their own homes.**About the Role:**As a Care Coordinator you will be an exceptional leader, the mark of your success is to make a positive difference to people's lives. You will spend each day assisting in the day-to-day operation of the Support Services, business and participate as required in multi-disciplinary assessment of service user needs.The successful Care Co-Ordinator will be responsible for ensuring all care plans are reviewed on a 6 monthly basis, spot checks are completed on a 3 monthly basis, staff are reviewed quarterly, and staff annual appraisals are completed.This is an office-based role in Wishaw and you will be required to commute to services in and around Wishaw and Coatbridge. You will be able to claim millage when out completing any reviews at 25p per mile, from office to service user and back to the office.**Role Responsibilities:*** Direct line management for a team of Support Workers, including the management of their weekly workload* Defining special care needs based upon identification of specific clinical problems and to monitor and review as appropriate* Liaising with external organisations in respect of developing a total package of care for a service users* Reporting to a Branch Manager on a weekly basis on activity levels and outcomes for service users* Providing written reports of all complaints, accidents, incidents and untoward events in accordance with written policies* Undertaking “on call” duties as required by the branch to ensure a 24/7 service is provided. *This will be on a rota basis 1 week at a time 1 out of every 4 weeks.***Key Criteria:*** Care Sector experience* SVQ 2 or higher or the willingness of completing SVQ 3* Experience as a Care Co-Ordinator would be preferred**Benefits:*** Cost of PVG covered* Family friendly policy to include enhanced maternity/paternity leave and much more.* Care Worker of the Month financial award* Refer a friend scheme (total award up to £1000)* Excellent training* Pension scheme* Cycle to work scheme* Eye test vouchers* Mears Fun Day - Our annual Fun Day is organised as a massive thank you from the Executive team for all the hard work Previous years include, Alton Towers, Drayton Manor etc.* Staff perks with Mears Rewards - discounts of up to 10% on coffee, weekly groceries, holidays, the list is endless.* Volunteering Leave - Mears supports employees to undertake volunteering in the community, in support of our social value commitment.**All our roles require candidates to have the entitlement to work within the UK, Mears does not currently offer visa sponsorship.****Candidates should be aware that all our roles are subject to relevant Background, Identity & Security checks before commencement of employment.**Apply below or to discuss your application further; contact:Vickie Rudge (Vickie.Rudge@mearsgroup.co.uk)If you need any help with your application process, we are here to support you.
